The Online Self-Management Program is made available Province-wide through a partnership between the Champlain Local Health Integration Network, Living Healthy Champlain, Bruyère Continuing Care, and the Regional Chronic Disease Self-Management Programs. It's goal is to help improve patient self-management of chronic conditions, improve health literacy and provide the opportunity for individuals living with chronic conditions to take control of their health and live healthier and more fulfilling lives.
